Our highly experienced and committed hospice and home health care team of medical professionals, social workers, clergy and support staff provide the following services:
• Hospice Care:
Hospice care is focused on providing comfort, not cure. It is a service for patients with terminal or life-limiting illnesses. We work with each patient’s primary care physician to provide a plan that is specific to the patient’s unique needs. Our specialized care is provided in a range of settings.
• Home Health Care:
Home health care is all-encompassing. Seniors with chronic illnesses or disabilities can benefit from the medical and/or therapeutic services provided by our licensed professionals. Home health care also includes domestic services as needed, and is available in the patient’s home or any place of residence.
• Palliative Care:
Palliative care is focused on relieving or controlling your pain. Our medical professionals and other staff work together to treat your specific needs, and those of your family members. Palliative care improves your comfort and your quality of life.
As a further level of convenience to our Hospice patients, we deliver medicines directly to you at home. Your medication will never run out, and your family will not have the additional concern of making trips to the pharmacy.
• Medical Equipment:
Heartwood Home Health and Hospice provides a variety of medical equipment to suit the needs of our patients who receive in-home services. The medical equipments are not limited to, but include wheelchairs, patient lifts, adjustable electric beds and oxygen concentrators.
• Private Duty Care:
Private duty care involves medical, domestic and/or respite care within your home. This service can include daily nursing visits, transportation, light housekeeping, meal preparation and recreational and other duties.
