Case Managers(Nurses)

Heartwood’s Nurses each have on average over 30 years of experience, coming from various backgrounds specializing in geriatric and palliative care. With this experience you know that you or your loved ones will be taken care of with the upmost respect and dignity. Case managers work with your doctor and Heartwood’s Medical Director to create a plan of care. The Case manager also works with the patient to develop the plan of care. The plan of care will be designed to monitor the patient’s condition and provide education to patients and caregivers. The case manager offers medical equipment and pain management medication as needed for comfort care. A Heartwood hospice nurse is available 24-hours a day, 7 days a week to patients and caregivers.

Certified Nurse Assistant’s(CNA’s)

Heartwood’s Certified Nurse Assistants are trained in geriatric and palliative care. Our CNA’s work with the Case Managers and the Plan of Care to best assist you or your loved one with their personal care including bathing, dressing, feeding and day to day activities using a caring touch. Due to the frequency of visits Heartwood’s CNA’s are usually develop a strong emotional bond with the patient and family. The CNA’s also offer support and education to the family.

Social Worker

Heartwood’s Social Workers are licensed to practice in the State of Utah and trained for geriatric care. They provide emotional support, education, grief and bereavement counseling for the patient and family to ease fears and help patient and family coping. Our social workers will also assist with Medicare/Medicaid paperwork, filling out advanced directives, finding support groups, help with financial assistance and funeral planning. Our Social Workers will go above and beyond to guide you through this life changing experience.


Heartwood’s Chaplain offers spiritual care for the patient and their family according to their wishes. Our Chaplain is experienced in discussing and supporting in the end-of-life journey however you or your loved one wishes. Our chaplain also can coordinate with the patient’s own clergy or religious leader.

Dr. Ermita Bort

Dr. Bort is devoted to helping provide patients the highest level of quality end of life care. She has over 30 years experience. Attending Vilnius University School of Medicine and serving her residency at University of Utah Medical Center.
